On Mya's Warrior Wednesday, instead of our weekly Warrior Shout - we are looking for your help. In support of The Frances Foundation, and in connection with follow-up as to the # of incidents of children residing in HOWELL, NJ being diagnosed with cancer over the last 5-10 years, TMLTF is soliciting the following information for all pediatric cancer patients/angels, to be part of potential pediatric cancer cluster study pursuit:
If you are willing and a present/former resident of Howell, NJ, via email to Kellylynnterry@msn.com, please kindly provide:
- Full Name of Child
- As parent/caregiver, your full name and contact information
- Diagnosis
- Date of Diagnosis
- Treatment Facility (Name and Location)
- Residential address
- If your child is an Angel, and the date of Gaining Wings
- Written consent to disclose all collected information, on your child’s behalf, to appropriate governmental and/or medical agencies.
